About the Role
Support Coordinator position available located in our Union Street, Newcastle centre on a permanent full time basis. In this position you will assist NDIS participants in the support coordination and implementation of all supports in their funded plan, including informal, mainstream and community supports, as well as funded supports.

Key Responsibilities
• Strengthen and enhance the customer's abilities to coordinate supports and participate in the community
  • Ensure the customer's plan is delivered within their plan budget
    • Ensure mainstream services meet their obligations to the customer (i.e. housing, education, justice, health)
    • Build the capacity of the customer to achieve greater independence to self-direct services and supports in the longer term
    • Provide the customer and the NDIA with reports on outcomes and success indicators, within the agreed reporting frequency

    Skills & Experience
    • Relevant Tertiary qualifications including a Certificate III or higher in Disability or equivalent or a Certificate III or higher in Mental Health
    • Demonstrated experience in working with people with a disability
    • Demonstrated experience in facilitating personal planning processes
    • Demonstrated application of the objectives and principles of the National Standards for Disability Services
    • Excellent verbal communication skills and a demonstrated ability to engage with clients and their families
    • Ability to think creatively and use initiative
    • Established problem solving and conflict management skills
    • Strong time management skills in a high volume work environment
    • Strong written and computer skills

    Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
