About the role
Responsible for developing and maintaining positive and effective relationships within the coordination of Out of Home Care services and programs with children, young people, families and other stakeholders. This position is part of a larger care team and is responsible for assessing, planning, monitoring and reporting on service delivery to new and existing children and young people and carers.

This role is full-time, permanent and based out of our Kalgoorlie Office

Key Responsibilities
  • Assess referrals and coordinate the analysis of support need, planning and program development in consultation with Care Team
  • Develop and maintain functional and supportive relationships with children and young people in out of home care to promote positive outcomes
  • Coordinate the transition of children and young people into care in consultation with Care Team
  • Ensure effective communication around Life Without Barriers service provision with carers, families and key stakeholders including government agencies
  • Provide after hours on call support to teams when required
  • Provide leadership, modelling and mentoring for members of the Care Team
  • Recruit, assess, train and support primary and respite carers to ensure all children and young people receive appropriate and quality care and ensure carer agreements, probity and trainings are current
  • Ensure all paperwork, file notes and other records are kept up-to-date, including inputting information into the CIRTS database.
  • Maintain and update professional skills by actively participating in personal, professional, service development and training opportunities
Skills & Experience
  • Tertiary qualification in Human Services or related discipline, or demonstrated extensive experience in a similar role
  • Experience of working in a leadership role in a human services organisation is highly desirable
  • Knowledge of working with children and young people within a State Legislation framework
  • Current Senior First Aid Certificate is desirable
  • A current unrestricted Driver Licence
  • Excellent time management and organisational skills
  • Excellent written and computer skills for the creation of plans, programs, reports, correspondence and other documents
  • an ability to build relationships with diverse cultural groups and to work within frameworks considerate of cultural needs

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
